Event Request Form

Thank you for wanting to host your event at LifePoint Church. There are a few things we would like you to be aware of:

The usage of the church is for LifePoint attendees only.
Events & event set-up cannot be scheduled on Sundays before 1 PM.
We kindly ask that all areas be left clean and organized after your event. This includes taking out the trash and and leave any kids’ areas neat and tidy if they’re used.
Please make sure any decoration you put up does not damage the walls and is removed after your event.
You are welcome to use the coffee equipment but please provide your own supplies (coffee beans, cups, sugar, creamer).
